COUNCIL EXPECTATIONS:
To remain an active member of this group each member must attend a minimum of one meeting per month. Following three unexcused absences the member will be removed from the group. Members must be prepared to volunteer for at least four hours/month as required for event and planning purposes.



Volunteer Confidentiality Agreement

I understand that the Canadian Breast Cancer Foundation is committed to protecting the privacy of personal information in its possession and that this information will be kept confidential and will not be sold, traded or loaned to any other organization. The Foundation will use your information only for follow-up contacts related to your position on the council. By providing this information you consent to the collection and use of the information you provide. I understand that the Canadian Breast Cancer Foundation may contact me with information regarding other Canadian Breast Cancer Foundation volunteer activities. If at any time I no longer wish to be contacted by the Canadian Breast Cancer Foundation, I may request to be removed from future mailings by calling the toll-free number listed on the front of the volunteer application form. I agree to respect the confidentiality of all information I may have access to at the Foundation. As a volunteer of the Canadian Breast Cancer Foundation, you may have access to information and documents relations to participants, donors, volunteers, and staff that are private and confidential in nature. All records are the property of the Foundation and will be treated as confidential material;
reasonable care and caution should be exercised to protect and maintain total confidentiality. Neither volunteers nor staff will give medical advice (including comments and suggestions that personalize medical information and influence treatment decisions), but may give information about breast cancer (consisting of facts available to anyone seeking general knowledge about the disease and its treatment).
